Our Community Health Team is here to provide additional services and support to families who have established care with us. The team consists of two social workers, a community health navigator, and a Psychiatric Nurse Practitioner. Together they ensure patients receive the best possible support, resources (e.g., access to transportation, housing, food, support groups), education, and mental health services.
