Pre-Admission/Advance Directives

Thank you for choosing Valley Regional Hospital and its team of healthcare providers. Upon entering VRH, the Admitting Department staff will ask you to verify pre-exiting information. Even if you were treated within the current year it's very important for us to have your correct information, including your current insurance. This insures that we are able to get medical results to your provider as quickly as possible.

When you come in for any Outpatient services, diagnostic testing, or surgery, please bring the following items:

  • Insurance Cards
  • Picture ID

If you would like to Pre-Register for any Outpatient Diagnostic testing, you may call Admitting @ 603-542-1833. When you arrive for services, please bring your two forms of identification.

Also during the registration process, staff may ask if you have completed an Advanced Directive. Advance Directives are instructions to your healthcare providers to be used at some future time when you are unable to communicate your wishes for care. Specifically, Advance Directives express your beliefs and wishes on important issues related to "lifesaving" care.

The best time to create your Advance Directive is before you enter the hospital, and after a careful discussion with your family and/or friends who will be in charge of your decisions and care.
