CVHC Tele-health Tele-health is a program the monitors the patient's vital signs and other symptoms to be observed through an in-home monitoring system. CVHC's clinical staff can access the data on a daily basis or more if needed via our electronic record system.

The Tele-health program allows patients to become better educated in their disease process, monitor their vital signs and weights, monitor their medications, provide reminders of scheduled activities, as well as increasing their independence in their care.

This daily monitoring of patient progress provides immediate access to the patient's health status that can be shared with physicians and family members when needed. A patient's daily interactions with their Tele-health monitors allow them to have consistent reinforcement about their disease status for improved outcomes.
