Skilled nursing services +

Skilled Nursing provides licensed nurses to assess, plan, monitor, treat and evaluate patient care needs.

Physical Therapy, Occupational Therapy, and Speech Therapy +

Occupational Therapy evaluates functional level, teaches therapeutic activities, fits self-help devices, adapts home environment and provides adaptive equipment recommendations.

Physical Therapy Services include the use of evaluation and assessment, therapeutic exercises, massage, heat, cold or electrical treatments and home adaptation by a physical therapist as part of the treatment plan.

Speech Therapy Services focus on assessment and treatment for those with an illness or injury, which affects speaking and hearing abilities, language skills or swallowing

Home Health Aide Services +

Home Health Aide Services and personal care services allow clients to have the daily assistance that is required to stay safely in their homes. Connecticut Valley Home Care & Hospice's Home Health Aide Services provides personal care such as bathing, dressing, mobility, nutrition, and medication. All of our home health aides follow a plan of care written by the registered nurse. CVHC's home health aides have been certified by the New Hampshire Board of Nursing as licensed nursing assistants certified to perform medication assistance in the home care setting.

Medical Social Work +

Medical Social Services include psycho-social support and planning assistance for the individual and family regarding limitations of an illness or injury as well as assistance and coordination of additional required resources. Medical social workers explore current options, helping individuals make difficult decisions. They consider financial implications and provide emotional support, enhancing the coping skills of the family or other caregivers

Psychiatric Nursing services +

Psychiatric Nursing Services provide licensed nurses, specializing in the care and treatment of individuals diagnosed with psychiatric illness.

Maternal Child Health Services +

Maternal Child Health Services provide licensed nurse services, specializing in the care, assessment, teaching and monitoring of newborns and mothers as well as children through their developing years.

Clinics, i.e. blood pressure, foot care, immunizations

Alzheimer's support group +

This group provides education and support to family members and caregivers of the Alzheimer's patient. The group meets the second Tuesday of each month from 6:30 to 8:00 p.m. in Buckley Conference Room of Valley Regional Hospital. The goal of home health services is to offer quality care in a safe home environment with support, assistance, and attention to the individual's personal, emotional, and physical needs. The primary clinician directs a team of home care professionals who work together to help clients gain strength, ability, and independence. With education as a key factor, our home health providers teach clients and family members the most effective ways to care for their medical and personal needs in order to regain their independence.

IV Therapy +

Intravenous Therapy Services provide nursing care of peripheral, central and P.I.C.C. lines, instruction to both patients and caregivers, and management of all types of pumps for hydration, hyperalimentation and medication administration such as antibiotics, chemotherapy and pain management

Wound Care +

Wound Care Services provide licensed nurses certified in the specialized care of individuals with recent surgical wounds, abdominal stomas, fistulas, drains, pressure ulcers, and/or continence disorders. As specialist, consultant, and treatment provider the wound care nurse along with the physican plays a pivotal role in the guidance of optimal patient care.

Homemaker/Companions +

Homemaker/ Companions/personal care services
CVHC's homemaker and companion program provide services to elderly and disabled clients in need of assistance to remain safe in their homes. The type of tasks provided include:
  • light housekeeping
  • laundry
  • meal preparation
  • groceries and other errands
  • companionship
  • light personal care
  • transportation to medical appointments for approved clients
We provide companionship to many isolated elders and individuals that live alone with very little community interactions. All homemaker services have individualized plans of care for each client and the homemakers document progress notes for each client as well as the supervisor of elderly services.

Historically, our homemakers have reported many health and social problems for these clients that needed intervention from professional medical staff. Without these "eyes and ears," many of potentially serious medical issues would have gone unnoticed until a more critical crisis developed. Connecticut Valley Home Care & Hospice's experience with providing homemaker services has been highly recognized by our local providers and clients.

Insurance coverage questions
Medicare and most insurance plans cover the cost of home care. Your individual insurance plan may have deductibles, co-pays, or limits. A representative of Connecticut Valley Home Care will help you understand the specifics of your insurance coverage.
