CVHC Adult Medical Day Program The Adult Medical Day Program is staffed by skilled, caring, and dedicated professionals providing personalized care and attention. Adult Medical Day Services works cooperatively with you and your doctor, family members, caregivers, and case managers to meet your specific needs whether they are medical, therapeutic or social. Our staff of nurses, licensed nursing assistants and an activities director works as a team to best meet your needs.

We are the perfect balance between healthcare and socialization in a safe environment for people between the ages of 18 and 100+.

The Adult Medical Day Program specializes in:

  • Holistic caring for people with physical, mental, social and spiritual needs
  • Creating a learning and sharing environment through interaction and engagement with staff and other clients
  • Case management with ongoing assessments and supervision
  • Being an integral element in the continuum of care
  • Ensuring that people remain as independent as possible
  • Helping vulnerable people remain active in the community
  • Providing respite care for families and caregivers

Daily therapeutic activities include:

  • Music, singing, dancing
  • Arts and crafts
  • Quilting
  • Games
  • Exercises
  • Hair and nail care
  • Making new friends
  • Reminiscence therapy
  • Baking
  • Gardening
  • Parties
  • Field trips

Health services include:

  • Skilled nursing care
  • Personal care
  • Medication management
  • Nutrition education
  • Wellness management

Meals Each day we provide a family-style breakfast, lunch, and afternoon snack.

Cost Program costs are covered by Medicaid, HCBC, ADRD grant funding, and Title XX funding. Some private insurance companies will also assist with costs. In addition, Adult Medical Day Services has a contract with the Veterans Administration to provide services to veterans. We can arrange for private payment plans.

Join the program Program participants may be referred by physicians, case managers, family members or friends; however, all admissions must be approved by physicians. Attendance ranges from one to five days a week. Please call (603) 543-6895 for more information.

The Adult Medical Day Program is a division of Connecticut Valley Home Care, a program of Valley Regional Hospital. The Adult Medical Day Program is licensed by the State of New Hampshire and the Department of Veterans Affairs, and is a Charter Member of the New Hampshire Adult Day Services Association.
